Practice Policies and Guidelines
Welcome to Lumina Mental Wellness. This page contains important information about this practice and its professional services and business policies. A copy of the information contained here will be presented to you and require your signature upon scheduling your first appointment. Please read it carefully and feel free to ask any questions you might have or for clarification of any policy.
-
Prior to your initial appointment, you’ll be asked to review and sign a consent form authorizing Lumina Mental Wellness to treat you or the minor for which you are the parent or legal guardian.
-
In compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPPA), we have adopted a HIPAA Notice of Privacy Practices. This document describes in detail how information about you, the patient, can be used within our office and with others who need to know the reason for treatment, payment, and/or health care operations. Lumina Mental Wellness provides a copy of the HIPAA notice to all patients. You should carefully review that notice for information regarding how your medical information may be used and disclosed, and how you can get access to this information.
-
At Lumina Mental Wellness, we are committed to providing high-quality, personalized care to each of our patients. In order to ensure adequate time and attention for every individual, we operate strictly by appointment only.
We are unable to accommodate walk-in visits under any circumstances. All services must be scheduled in advance through our online booking system.
This policy helps us maintain a calm and organized environment while respecting the time of both our patients and the provider.
Missed Appointments
The efficient operation of this practice requires that patients arrive on time for scheduled appointments. Failure to keep appointments interferes with Practice operations and the delivery of quality care. When a patient misses an appointment without cancelling it within 24 hours in advance it is considered a “Missed Appointment.”
Lumina Mental Wellness will directly charge your METHOD OF PAYMENT on file for the FULL COST OF THE SERVICE that was scheduled.
We understand that there may be exceptional circumstances beyond a patient’s control that lead to a missed appointment. In such cases, we maintain the discretion to waive or adjust the fee.
-
Prior to your first appointment at Lumina Mental Wellness, you will agree to accept responsibility for payment for all services and products received.
You may pay by cash, check or credit card, and we encourage all patients to maintain an active method of payment on file for ease of future billing.
Lumina Mental Wellness will charge an additional processing fee if your copay and/or deductible are not met at the time of service. There is also a processing fee of $50.00 for any returned check or collection on accounts.
If you opt not to maintain a method of payment on file, you will not be able to schedule any telehealth services, phone consultation services or utilize the patient portal’s messaging function.
We are currently accepting Aetna, Ambetter Marketplace, BlueCross BlueShield, Cigna, United Healthcare, Medicare, Medicaid (Magnolia & CHIPS) and self-pay patients.

Lumina Mental Wellness may communicate with you through a variety of electronic methods, including but not limited to SMS/text message, email, and voice messages.
Lumina Mental Wellness also utilizes AI technology to capture and transcribe discussions that take place during your appointment so that the provider may focus on you in lieu of typing notes.

IF YOU ARE EXPERIENCING A MEDICAL OR PSYCHIATRIC EMERGENCY, PLEASE CALL 911 OR GO TO THE NEAREST EMERGENCY DEPARTMENT IMMEDIATELY.
Please do not leave urgent messages on voicemail. Voicemail is not monitored after 5:00 PM Monday through Friday, on weekends, or on holidays.
-
Lumina Mental Wellness adheres to the following prescription refill policies:
All prescription refill requests must be submitted through the Patient Portal.
Requests submitted via phone, text, or any other method will not be accepted.
Refill requests must be submitted at least seven (7) business days before the medication is due. This allows adequate time for review and processing.
Missed Appointments: If the need for a refill arises due to a missed appointment, the patient will be subject to a $50 refill request fee. Only enough medication will be prescribed to last until the rescheduled appointment, and no refill will be sent until that appointment is scheduled.
Controlled Substances: If the refill request is for a controlled substance and the patient has not completed a urine drug screen (UDS) within the past 90 days, a UDS will be required before any controlled substance prescription can be refilled.
The cost of the urine drug screen, missed appointment, and the refill fee are the patient’s responsibility.
-
Telemedicine is the remote evaluation and treatment of patients by means of telecommunications technology. After an initial in-person consultation, you may be eligible to receive telemedicine sessions if your health care provider determines it could benefit you. You have the right to withhold or withdraw your consent for the use of telemedicine at any time. Your health care provider may also determine that your telemedicine sessions should be terminated in place of in-person care.

At Lumina Mental Wellness, we are committed to providing ethical, evidence-based psychiatric care within the scope of our clinical practice. However, there are specific services that are not provided through this practice.
The following services are excluded and will not be offered:
Custody Evaluations – including parental assessments intended for use in determining custody or visitation rights.
Forensic Evaluations – including psychiatric evaluations requested for legal proceedings or court- related purposes.
Court Appearances – including testimony, depositions, or letters intended for use in legal cases.
Substance Abuse Treatment – including intensive outpatient programs, detoxification, or primary substance use counseling services.
Disability Evaluations – including completion of forms for Short-Term or Long-Term Disability, or documentation supporting extended leave from work or school.
If you require any of the services listed above, we recommend that you seek assistance from a provider or agency that specializes in those areas.
-
The decision to discontinue treatment is ideally made through a mutual and thoughtful discussion between the provider, the patient, and—when applicable—the patient’s parent or guardian.
However, if you choose to discontinue treatment without notifying Lumina Mental Wellness, it will be assumed that your therapeutic relationship with your provider has ended 90 days after your last completed appointment, unless you have a future appointment already scheduled.
After that 90-day period, your provider will no longer be responsible for your psychiatric care, including but not limited to medication management, refills, or documentation requests.
If you wish to resume care after this period, you may be required to complete a new evaluation and intake process.
-

A patient exhibits violence, verbal abuse, carries a weapon on or engages in illegal acts on the premises.
A patient refuses to comply with stipulated program rules, refuses to comply with treatment recommendations, does not provide appropriate forms upon initial treatment, or does not make payment or payment arrangements in a timely manner.
A patient does not attend scheduled appointments for 2 consecutive sessions without notifying Lumina Mental Wellness that they will not be attending at least 24 hours in advance of the schedules appointment.
